Client Forms
I typically will ask clients to read the Psychotherapy Agreement prior to the first session. The Psychotherapy Agreement contains basic information about how therapy with me will work and each of our rights and responsibilities. You can print out and sign the Acknowledgement form to show that you've read, understood, and agree to the Psychotherapy Agreement.
Similarly, I usually ask clients to fill out the Intake Form prior to the first session. This provides me with basic information about you (how to contact you, emergency contacts, etc.) as well as some information about your history and what brings you to therapy.
Sometimes it may be useful for me to speak to another person or group about your therapy (such as your physician, or a prior therapist), at which point I will ask for a Release of Information form. Without this signed form, I am not legally able to discuss your therapy with anyone else.
